At Associates for Human Development, our clinical staff is comprised of licensed therapists who are formally educated, trained and experienced in a wide range of issues, from interpersonal and relational matters to behavioral and mental disorders.
Our counseling services include, but are not limited to:

- Overcoming anxiety, depression, low self-esteem, a sense of failure or lack of fulfillment

- College preparation and/or adjustment issues
- Issues related to separation, divorce, remarriage and blended families

- Geriatric and caregiving concerns
- End-of-life care

If you're a first-time client, please review and complete the forms below. Be sure to bring them to your first session so we may put them in your confidential chart.
If you are attending as a couple, we ask that each party complete and submit a copy of the forms from their own perspective.
If client is under 18, we ask that the parent complete the New Client Information in the parent’s words, and the parent or minor completes the New Client Background in the minor’s words.
